P was left alone in aviary for an hour and did not move from spot in corner. When staff member went to catch p it flew up to a perch, missed slightly, but corrected and was able to land on perch. Was also able to fly to avoid people in the aviary trying to catch it.

Notes:
Consistent with visual deficits, fluttering to the top of aviary, bumping into the ceiling. Not going toward perches at all. Fell down to the ground in back corner and then did not move. Turns head side to side likely tracking sounds.

Neurology Consult:
No CN deficits were appreciated. Based on appropriate mentation, neuro suspected problem is more likely peripheral (otitis interna or media). Other differentials include CN 7, 8 abnormality, trauma affecting the brain (infectious or inflammatory),or a brain lesion.

We discussed pursuing CT with and without contrast and the pros/cons of each.
Without contrast we are looking for a lytic lesion or a mass effect.
With contrast we have the risk of the bird having an adverse reaction to the contrast and dying, there is also a worry about the bird staying warm during the CT. But that if want to see anything vascular we would need contrast.
Overall major issue will be that the bird is so small that there will likely be reduced resolution and detail.

Neurological: upright posture, L head tilt ~15 degree at rest, does become normal when stressed
CN: odor ?, menace neg OU, direct PLR + OU, no strabismus or nystagmus, + palpebral, OS blepharospasm vs ptosis, possibly asymmetric expression with plumicorns, + startle/balance, taste?, + gag and tongue grab
Postural Reactions: CP wings and legs wnl, drop flap wnl, hopping neg, tactile placing +L -R, visual placing neg, good balance on glove and flat surface
Spinal Reflexes: vent sphincter tone, limb withdrawal wnl
Sensory Response: wnl
Other observations: when placed back in cage p did hop from top of box to perch accurately, did seem to react to some movement but difficult to rule out reaction to sound, when stressed did have both eyes fully open and no head tilt, but then once stopped stimulation the head tilt and partial closing of L eye returned

Problem list:
- suspected central visual deficits OU
- intermittent head tilt and partial closure of L eye
- not eating well past few days, regurgitated last night

Diagnosis: undetermined, suspect brain lesion secondary to trauma

Plan: Could consider neuro consult, but unsure that examination alone is likely to lead to any changes in dx/tx. Could consider brain imaging, though with patient's small size we may not be able to get useful diagnostic images with CT. This also may not actually affect our treatment plan. Can give more time in care, though some parameters are changing for the worse.

Prognosis: Extremely guarded to poor

Ophthalmic examination was indicated for 003 after flight evaluation. On flight evaluation the patient seemed non-visual and would continuously fly up toward the top of the avairy.

003 Ophthalmic examination's main finding revealed an incompletre and slow PLR in the right eye.It is suspected that the right eye has a retinal tear/degeneration. The significant findigs of the left eye include an incipent cataract.

OD in depth findings: 2-3+ mydriasis present, hazy view, blunted corpora nigra, large linear white streak doraslly with focal dipigmentation periphery of pecten.

OS in depth findings: focal pinpooint white opacity axially, incipient anterior subcapsular axially.

No recheck is needed for 003.
Plan is to consider neurology consult for visual deficits.

The vestibular signs and evidence of blood in the mouth/nose are suggestive of head trauma. Patient condition is not critical enough to warrant stress of IO catheter placement so oral/SQ medications are elected at this time. Meloxicam may reduce inflammation of the CNS and buprenorphine will help control pain.
